It's a weird feeling when you try to seek advice about implementing GDPR from a company that obviously doesn't care about implementing GDPR. The form takes SWATHES of personal data and gives no indication what it will be used for (no, having a privacy policy link somewhere on the page is not sufficient anymore).
So the only consent that can be inferred is that the e-mail address will be used for delivery (except it isn't: you get a download link by e-mail which again goes out to *somewhere*) but there's no sane reason you'd need the first name, last name or phone number for that.
Something tells me you're probably storing that information, though, and actively using it for something other than merely sending out e-mails with links. THAT'S FINE, but you need to disclose that explicitly in advance so people can give informed consent. This form is exactly the kind of thing that would get a company reported to a supervising agency for a GDPR violation.
